Personalized care together with advanced technology is what it means to be Community Strong Responsibilities The Project Coordinator is responsible for enhancing SJRH's capacity to provide opt-out HIV testing and to implement opt-out testing for Hepatitis C in our Emergency and Inpatient setting up systems to expedite testing, including enhancements to the EMR and making testing more patient-friendly. Develop systems to link & track eligible individuals to HIV treatment, prevention, and Hep C treatment through the hospital's EMR system. Work in collaboration with the hospital's ED/Inpatient staff to develop an efficient workflow and patient centered interactions, to expand the rate of patients receiving HIV & Hepatitis C testing in these areas. Develop workflows that enhance referrals for linkage to care within the hospital's existing system or community providers. These workflows will be for individual who are found to be HIV positive and not in care, HIV at risk (and not in care) and those who are Hepatitis C infected and in need of treatment services. Work with the hospital's Analytics, IST, and Informatics departments to enhance the Electronic Medical Record (EMR) to support access to data to enhance testing and linkage. Implement specific spreadsheets and/or databases to enhance tracking of patients who are linked to care. Update organizational policies to support universal testing and linkage services. Support quality improvement efforts to enhance and streamline internal processes for universal testing and linkage services. Introduce systems to link HIV-negative and at-risk individuals to existing Comprehensive Prevention Services within the hospital system or community provider. Ensure all grant deliverables are met and that the program is meeting the funder's Scope of Service. Develop methods of collecting monthly and quarterly data elements and reporting these required data elements, as required. Work collaboratively with the Project's Principal Investigator/Project Champion to implement updated systems in the Emergency Departments and Inpatient Services and on the development of data driven presentations. Supervisor Patient Navigator and Data Analyst. Performs unrelated duties in times of emergencies and assumes other responsibilities when assigned. Qualifications Master's in Public Health, preferred. (Masters in Social Work will be considered, based on applicant's Bilingual: Spanish/English is preferred
